Dr. Simon Ourian, a prominent figure in cosmetic dermatology, has gained fame through his Beverly Hills clinic, Epione, and a clientele of A-list celebrities. His rise to prominence, largely attributed to endorsements from the Kardashian family, has made him a sought-after “celebrity dermatologist.” However, beneath the glamorous facade lies a complex and controversial professional history that raises significant questions about qualifications, practices, and patient care in the field of cosmetic dermatology.
Epione, established in 1998, markets itself as a cutting-edge aesthetic laser surgery clinic. Ourian has also launched Epione Skin Care, purportedly developed to address a perceived lack of effective skincare options for his patients. While these ventures have been commercially successful, they have also been subject to intense scrutiny from medical professionals and former patients alike.
Cosmetic dermatology, Ourian’s field of practice, focuses on aesthetic procedures such as skin tone alteration, fat reduction, and wrinkle smoothing. Typically, practitioners in this field require board certification, which includes completing a dermatology residency. However, investigations into Ourian’s background have revealed that he lacks formal dermatology residency training, instead having a background in anesthesia and internal medicine.
This lack of specialized training is particularly concerning given the complex and potentially risky nature of many cosmetic procedures. A 2022 study published in the Journal of the American Academy of Dermatology found that nearly 40% of providers advertising themselves as “dermatologists” on Instagram in certain metropolitan areas were not board-certified dermatologists, highlighting a broader issue of credential misrepresentation in cosmetic medicine.
Ourian’s professional history is further complicated by significant legal issues. In 2005, he faced 29 charges from the state of California, including gross negligence and aiding unlicensed medical practice. These charges led to the revocation of his medical license in 2009 and a subsequent five-year probation period. During this time, Ourian was required to complete additional medical education and could only practice under the supervision of a board-certified doctor.
Despite these setbacks, Ourian managed to rebuild his career through celebrity connections and media appearances. However, his practices continue to raise concerns among medical professionals. Dr. Adam Rubenstein, a board-certified doctor, has criticized Ourian’s use of unconventional treatment names and questioned the results of his procedures. Treatments like “Lipo Freeze” and “Cool Laser” have been scrutinized by other professionals, including Dr. Jason Emmer, who reported finding no positive effects from “Cool Laser” treatments.
The use of proprietary terms for treatments is a growing concern in cosmetic dermatology. A 2023 review in the Journal of Cosmetic Dermatology highlighted the proliferation of trademarked procedure names, noting that they often obscure standard techniques and can mislead patients about the novelty and efficacy of treatments.
Patient accounts further illustrate concerns about Ourian’s practices. Multiple reviews describe unsatisfactory experiences at Epione, involving expensive treatments with disappointing results and potential side effects. These accounts align with other negative online reviews about Ourian’s treatments and patient care.
The case of Dr. Ourian is not isolated. Similar controversies have emerged surrounding other high-profile cosmetic surgeons. For instance, Dr. Michael K. Obeng, known for high-profile procedures, faced a lawsuit in 2021 for alleged negligence during a buttock augmentation surgery. Similarly, Dr. Raj Kanodia, a celebrity plastic surgeon in Los Angeles, has faced multiple malpractice lawsuits despite his prestigious reputation.
Research published in the Plastic and Reconstructive Surgery Journal in 2021 found that while social media presence and celebrity status of cosmetic surgeons often led to an increase in patient inquiries, these factors did not correlate with improved surgical outcomes or patient satisfaction rates. This study underscores the potential disconnect between a surgeon’s public image and their clinical competence.
The case of Dr. Simon Ourian and similar instances in the field highlight several critical issues in cosmetic dermatology and surgery. These include the prevalence of non-board-certified practitioners, the use of misleading proprietary treatment names, and the manipulation of online reviews. Such practices create a challenging landscape for patients seeking safe and effective cosmetic treatments.
In conclusion, the story of Dr. Ourian serves as a cautionary tale for prospective patients in the field of cosmetic dermatology. It emphasizes the importance of thorough research beyond celebrity endorsements and carefully evaluating a practitioner’s qualifications, track record, and patient reviews. Patients are strongly advised to seek board-certified professionals, obtain multiple consultations, and approach novel or trademarked techniques with informed skepticism.
As the demand for cosmetic procedures continues to grow, addressing these issues becomes increasingly crucial for ensuring patient safety and satisfaction. The case for stronger regulation in cosmetic medicine, improved transparency in practitioner credentials, and more robust patient education has never been more compelling. Only through such measures can the field of aesthetic medicine hope to balance innovation with patient safety and ethical practice.
Friend went to see Dr. Simon Ourian a couple of years ago. He botched the procedure and the scars are still there. When she complained about the problem, his staff acted as if it was her fault that the results weren’t what Simon had promised to her. This guy is a sadist.
Simon Ourian is a terrible doc. He has taken advantage of so many patients that it’s outrageous at this point. The medical board should conduct a detailed investigation and take away his license. That’s the only solution to this problem.
I did my best to get Simon Ourian to do fat transfer to treat my frown lines. He is all I could ask for, cause I knew he is very reliable. I absolutely love the result! The lines are gone!
In the clinic of Dr. Ourian, I had an operation to correct the shape of the lips. Asymmetry began to manifest itself not so long ago, but still became noticeable. Especially she appeared during a conversation or when I smiled. It became annoying to me, and I decided to correct this defect. I am pleased with the result. Everything was done neatly, exactly as I wanted. It gave me a lot of confidence. Thanks for the help!

I never even dreamed that I could admire my face again! Implantation is contraindicated for me, so I was looking for a specialist who would help me with other methods. I heard a lot about Dr. Ourian, but this time he surpassed himself. I don’t think it was easy, but you managed to improve the contours of my jaw and chin. “Strengthened” lines gave me more expressive features. My joy was no limit when I saw my “new” face. You do the impossible!
As a teenager, I suffered from acne on my face. But it was not without consequences – numerous scars ruined my appearance. At the Epion Clinic, I received more than a cosmetic procedure. Dr. Urian managed to minimize the most damaged areas by making them smooth and indistinguishable from healthy skin. I barely recognized myself! My face was so “bright” and tender only in childhood. This is an incredible feeling of joy! You managed the impossible!
Clean skin is my dream! Who has lived all his life with pigmented spots on his face, he will understand how it spoils the appearance. But the treatment was so simple and effective that I could not believe that I had changed so quickly. This “magic” laser in the hands of such a wonderful doctor did more than I could have imagined! I didn’t go to dr Simon. Can’t comment personally , but I think his prices are insane. And it’s not the question of you can afford it. It’s the question where it does cost that much? The fillers he uses are 10 times cheaper and you don’t have to be “god gifted” to do lips injections. Most of good cosmeticians (with degree of cause). And do it. And I’m sure either in LA or any other city there are many good ones. Usually at good clinic it cost 300-500$. And lasts 4-8 month. Best would be juvederm. There are no fillers that last 3 years!!! Radiess would probably be to longest 1.5 year. But he mixes it with hualuronic. Please make ur research. All fave treatments like lasers or thermal and etc are based on machines. And it’s important to put write program on and that’s all. It’s done by machines. Again u don’t have to be “god gifted talented” to move the tool on the clients face. Of cause you have to be educated! But that’s not rocket science. All celebrities who posts online have barter deal. But does it really cost that much? Definitely not! Plastic surgery yes because it’s forever. But hualuronic face filers? Definitely not!!!
